Background investigations include as applicable: criminal background, reference checks, and review of the past three years of your driving record provided by the Department of Motor Vehicles. Failure to receive satisfactory reports from the investigations and/or refusal to submit the investigations will be cause for disqualification.
I certify that the facts contained in this application are true and complete to the best of my knowledge and understand that, if I volunteer, falsified statements on this application shall be grounds for dismissal. I authorize investigation of all statements contained herein and the references listed above to give you any and all information concerning my interest to volunteer and any pertinent information they may have, and release all parties from all liability for any damage that may result from furnishing some to you. I understand and agree that if approved to volunteer, I will abide by all rules and regulations of The Children's Course. |
